Welcome to our referral page, here you will find out all you need to know about our referral system.

Self Referral

If you are referring yourself, a family member or friend please download our self referral form. (Having trouble downloading the document? Download the PDF version.)

If you are from a health or social worker, GP, nurse or other practitioner then please download our practitioner referral form. (Having trouble downloading the document? Download the PDF version.) For the befriending criteria click here and for the guidance notes please click here.

How long will a referral take?

Roar will process referrals as soon as possible and then if the referral is for a Health and Wellbeing club the paperwork is passed to the Club Coordinator who will be in touch to organise a start date.

If the referral is for befriending this may take a bit longer to organise as we currently have a waiting list and volunteers have to be matched to the most appropriate person to suit location, interests etc. A visit will be organised by Roar staff to speak to the older adult who requires the service and find out a bit more about them. They will then be matched as soon as we have a suitable volunteer. All our befriending volunteers will have been police checked.
